About the DFI test

Sperm DNA fragmentation refers to breaks or damage in the genetic material (DNA) carried by sperm. The DNA Fragmentation Index (DFI) is the percentage of sperm in a sample that have fragmented DNA.

Sperm can look normal in a routine semen analysis and still carry damaged DNA. High levels of DNA fragmentation have been associated in some studies with difficulty conceiving, poorer embryo development and miscarriage. At Skanda Fertility & Women's Health Centre, Thanjavur, the test is advised when the history suggests it may help guide treatment.

When the DFI test may be advised

The test may be useful in situations such as:

  • Unexplained infertility
  • Recurrent miscarriage
  • Repeated unsuccessful IUI, IVF or ICSI
  • Poor embryo development in previous IVF cycles
  • Varicocele
  • An abnormal semen analysis
  • An older male partner, smoking, or exposure to heat or toxins

Your specialist will advise whether the test is likely to be useful for you.

What can affect sperm DNA

DNA fragmentation can be influenced by factors such as:

  • Smoking and alcohol
  • Infection or inflammation
  • Varicocele
  • Fever or recent illness
  • Heat exposure
  • Increasing age
  • Excess weight

When fragmentation is high, the plan may include treating the cause, lifestyle changes, or choosing a particular treatment approach.

How is the test done?

It uses a semen sample collected in the same way as for a semen analysis. The sample is processed in the laboratory with a specialised technique that measures DNA damage.

What is a normal DFI?

Laboratories use their own reference ranges, but a lower DFI is generally better. Your specialist will explain your result together with your other tests.

Can a high DFI be improved?

In some men it can. Treating causes such as infection or varicocele, stopping smoking and other lifestyle changes may help, and a repeat test may be advised after a few months.

Does every man need a DFI test?

No. It is a specialised test usually advised in particular situations, such as unexplained infertility, recurrent miscarriage or repeated unsuccessful treatment.
